Booking
Book a visit by phone during the clinic's opening hours. Below you will find what to prepare and how to cancel a slot.
- 1Call reception89 646 72 46 or 793 842 057, Mon–Fri 7:00–18:00. Quietest between 10:00 and 12:00.
- 2Choose a doctor and a timeWe will give you the next available slot with the doctor you choose. Urgent cases are seen the same day.
- 3Arrive a few minutes earlyReport to reception with an ID card. For a child: the child's PESEL (national ID number) and a parent's ID.
What to bring
- The patient's PESEL (for NFZ entitlement verification via eWUŚ)
- A list of medicines taken regularly
- Test results, if done outside the clinic
- For a child: the child's health record book
Cancelling or changing a slot
Call reception at the latest the day before. A freed slot goes to another patient.
