What data we process on the website
The website nzoz-sanus.pl does not collect data through forms and does not use cookies for tracking.
- Server logs — IP address, request time, page address, browser. Legal basis: the controller's legitimate interest (security and maintenance of the service). We keep these for up to 30 days.
- Visit statistics — we use the Umami tool, which runs on our own server. It does not set cookies and does not identify individuals; the IP address is anonymised. Legal basis: legitimate interest (analysis of how the website is used).
- Google map — loads only after you click "Load Google map". Google may then set its own cookies in accordance with Google's privacy policy.
Phone and e-mail contact
Data provided during a call with reception or by e-mail is processed in order to respond and handle the visit. Medical data is processed in medical records under the legislation on healthcare activities and patients' rights, for the period required by law.
Recipients of data
Our hosting provider (a server located in the European Union) and bodies entitled under the law. We do not transfer data outside the European Economic Area, except for the Google map described above once it has been loaded.
Your rights
You have the right to access your data, to have it rectified, erased or restricted, the right to object, and the right to lodge a complaint with the President of the Personal Data Protection Office (Prezes UODO). Send requests to the e-mail address given above.
