Order a repeat prescription

For patients with chronic conditions who have a POZ declaration on file. The doctor issues the e-prescription within 3 working days. You will receive the code by SMS or in your Internet Patient Account (IKP).

  1. 1Call or ask at receptionGive your PESEL, the medicine's name, dose and quantity.
  2. 2The doctor reviews the requestThey check your records and issue the e-prescription.
  3. 3The e-prescription code arrives by SMSOr appears in IKP. You can redeem it at any pharmacy.

How to become our patient

The declaration of choice of doctor, nurse and midwife is free and can be changed twice a year at no charge.

1. Download it or fill it in on site

Get the declaration form at reception or download it from the list below.

2. Come with your ID

An ID card is enough. For a child: the child's PESEL and a parent's ID.

3. Ready straight away

Once it is entered into the system, you can book your first visit.

Questions and answers

Frequently asked questions

How do I submit a declaration to a POZ (primary care) doctor?

In person at reception (an ID card is enough) or online via IKP (Internet Patient Account) at pacjent.gov.pl. The declaration takes effect immediately.

Can I order a prescription without a visit?

Yes. Patients with chronic conditions can order a repeat prescription by phone at reception or via IKP. The e-prescription code will arrive by SMS or appear on IKP.

How do I cancel or reschedule a visit?

By phone at reception, at the latest the day before. This lets the slot go to another patient.

Do you see patients privately?

Yes, subject to availability. The price list for private visits is available at reception.

What should I do outside the clinic's opening hours?

At night, on weekends and public holidays, night and public-holiday out-of-hours care operates at the hospital in Ostróda, ul. Jagiełły 1. In a life-threatening emergency, call 112 or 999.
